Responsibilities

  • - Support the delivery of evidence-based recovery-focussed interventions
  • - Support individuals to achieve positive outcomes and recovery goals based on the multidisciplinary team assessment and formulation of the individual’s needs
  • - Engage the service users via in-reach into the prison/hospital before they are released/discharged into the community
  • - Continue to work with them in the community to promote recovery
  • - Work proactively with other professionals and services, such as GPs, substance use agencies, DWP, Housing providers, families & carers, friends and peer mentors to support recovery plans
