  • A vacancy has arisen for a Family Partnership Practice Lead (FPPL) working within the Tonbridge and Malling district.

  • The Family Partnership Practice Lead will have a key role in embedding the Family Partnership Model within Kent Health Visiting Services under the guidance of the Family Partnership Programme Manager.
  • The post holder will also be a clinical leader and provide teaching, mentoring and supervision to junior staff to ensure the effective delivery of the programme.
  • The Family Partnership Practice Lead will work in partnership with partner agencies in the identification and protection of vulnerable children and contribute to the development of care plans, working in accordance with local and professional guidelines.
  • They will ensure completion of agreed outcomes for delegated Children Protection cases as per agreed multi-agency care plan ensuring regular communication with caseload holder.

Responsibilities

  • The Family Partnership Practice Lead will facilitate the early identification of vulnerable families and support the delivery of the Family Partnership Programme by managing a small caseload of families, supporting them ante-natally and until their infant is a year old.
  • The post holder will have a lead role in embedding the Family Partnership Model within district health visiting practice as well as providing leadership and some line management to the district health visiting team.
  • The post holder will also be a clinical leader and provide teaching, mentoring and supervision to junior staff to ensure the effective delivery of the programme.
  • The Family Partnership Practice Lead will work in partnership with partner agencies in the identification and protection of vulnerable children and contribute to the development of care plans, working in accordance with local and professional guidelines.
