  • - Je hebt een agogisch mbo-4 of hbo-diploma (bijvoorbeeld MMZ, SPW, SPH, Social Work).

  • - Je bent een aanpakker en vindingrijk in het bedenkingen van oplossingen.
  • - Je staat sterk in je schoenen.
  • - Je spreekt en schrijft goed Nederlands.

Responsibilities

  • - Jij coacht mensen met een verstandelijke en/of psychiatrische beperking naar zelfstandigheid.
  • - Je coacht mensen met diverse problematieken en daarbij houd je rekening met de leeftijd en het niveau van de cliënt.
  • - Je werkt aan de hand van een ondersteuningsplan en rapporteert de resultaten in het ECD.
  • - Periodiek evalueer je en stel je indien nodig de doelen bij.
  • - Naast de cliënt ondersteun je ook zijn of haar (sociale) netwerk met advies en informatie.
  • - Jij kunt bij jouw collega’s terecht voor steun en advies.
  • - Je werkt volgens een onregelmatig rooster in dag-, avond-, en weekenddiensten en slaapdiensten.
