Responsibilities

  • An exciting opportunity has arisen for a suitably qualified Band 4 Assistant Practitioner to join the Newport community team in the provision of clinical care to individuals in their own home.
  • As Assistant Practitioner you will work with the wider community MDT under the delegation of your Band 7, Band 6 and a team of Registered Nurses. You will utilise and develop a wide variety of skills to meet the emotional, physiological, and clinical needs of individuals.
  • The post holder will work collaboratively with and support members of the multidisciplinary team in the delivery, development and management of health and appropriate referral for social care issues. The role is challenging but rewarding and you will be fully encouraged and supported with your professional development.
