Responsibilities

  • As a Band 6 Physiotherapist, you will provide an integrated Health & Social Care service for people at home who are at risk of being admitted to hospital unnecessarily, providing Community Rehabilitation, Rapid Response, Reablement and Supported discharge.
  • You will be working closely with our acute partners, such as Imperial College Healthcare NHS Trust and Chelsea & Westminster Hospital NHS Foundation Trust.
  • We will connect you with peers across the tri borough of Hammersmith and Fulham, Westminster and Kensington and Chelsea to support you and to work on service improvement partnerships.
  • We will ensure you are able to follow the clinical and leadership development priorities identified in your personal development and appraisal plan.
  • We will equip you for mobile working and actively support flexible working practice when required and appropriate for the service.
