  • Maintain effective communication with the MDT ensuring active participation with nursing and medical teams.
  • Maintaining personal contact with patients, relatives and carers and being sensitive to their needs for courtesy, dignity and privacy and ensuring a friendly environment at all times.
  • Ensuring that accurate and timely records of discharge planning assessments, interventions and contacts are maintained.
  • Maximising the utilisation of the electronic WhiteBoard, using it as a basis for everything in terms of safe discharge and transfer of patients.
  • Prioritise own workload and ensure effective time management strategies are embedded within the culture of the FEAT team.
  • To ensure all nursing or therapy procedures are carried out in accordance with Derby Hospitals NHS Trust policies taking appropriate action in emergency situations.
