Responsibilities

  • The main duties of the posts are to assess and review individuals with complex health care needs within the community and hospitals.
  • The role will require the post-holder to undertake in-depth expert / specialist nursing assessments in hospital sites across the five Locality areas of Gwent, on behalf of Aneurin Bevan University Health Board, to ascertain individual’s eligibility for Continuing NHS Healthcare (CHC), Discharge to Assess and entitlement to NHS Funded Nursing Care (FNC) funding.
  • Consequently, the role will require the post-holder to facilitate the Multi-disciplinary Team assessment process, and to provide input into the discharge process.
  • The post-holders will promote collaborative and inter-agency working across professional and organisational boundaries by providing professional advice, support and guidance to both professional and non-professional colleagues, patients, and their relatives / representatives.
