Responsibilities

  • The post offers opportunity to deliver a variety of assessments and interventions with patients in the hospital and on occasions in their own homes and communities.
  • The post offers opportunities to manage a caseload, develop knowledge, skills and experience in supporting patients to be discharged from hospital.
  • We need an effective communicator who can work flexibly and proactively to deliver a prompt and clinically effective service.
  • You should be a creative problem solver, and demonstrate a willingness to develop and pilot new ideas.
  • You will possess excellent organisational skills related to yourself and supporting the workload management of the team.
