Responsibilities

  • Exciting new opportunity to work in Angus within a brand-new, innovative service.
  • Redesigning the way we deliver care, focusing on a model that brings together multi-agency teams to prevent unnecessary hospital admissions and support people to remain at home.
  • Ensuring that individuals who do not require acute hospital admission or who are already in hospital but no longer need acute care are redirected and supported to receive the Right Care, at the Right Time, in the Right Place.
  • Contributing to the reshaping of community services through the Frailty at Home and Home First Angus Single Point of Contact initiatives.
  • Working closely with multidisciplinary teams, including Occupational Therapists, Enablement response teams and Medicine for the Elderly advanced nurse practitioners (ANP’s).
  • Weekend working as part of a shift pattern.
  • Travel throughout Angus is essential.
