Responsibilities

  • The post holder will be responsible for delivering a high-quality mental health service to patients under the care of the Crisis Assessment and Home Treatment Team (CAHTT) within secondary mental health services.
  • CAHTT is dedicated to providing timely, flexible, and person-centered interventions aimed at preventing hospital admissions and readmissions. The team focuses on supporting individuals within their communities, tailoring care to meet their unique needs.
  • The role involves actively contributing to the continuous development and improvement of services, ensuring that effective care pathways and individualised care plans are in place to facilitate urgent and crisis care access. The post holder will champion evidence-based practices by offering clinical advice and support, serving as a role model, and delivering education and training as appropriate.
  • Close collaboration with carers, statutory and non-statutory services, emergency responders, and third-sector organisations is essential. The post holder will also work closely with local Single Point of Access services to provide rapid responses to mental health crises, helping to reduce unnecessary A&E attendances.
